Waiver and Consent

I hereby give consent for Gold and Grace, LLC to perform piercing(s) and/or to provide jewelry custom-fitted and welded to fit my body specifications. I understand and promise to follow each step of the aftercare instructions (verbal or written), as provided by Gold and Grace, LLC. I acknowledge that it is my responsibility to follow the after-care instructions and consult with a medical provider if there are signs of an allergy, prolonged irritation, or other unexpected bodily reaction.

I HEREBY AGREE TO RELEASE AND FOREVER DISCHARGE AND HOLD HARMLESS GOLD AND GRACE, LLC FROM ANY AND ALL CLAIMS, DAMAGES OR LEGAL ACTIONS ARISING FROM OR CONNECTED IN ANY WAY WITH THE PIERCING(S), THE PIERCING PROCEDURE, THE JEWELRY OR THE JEWELRY WELDING PROCEDURE. I CERTIFY THAT THESE SERVICES ARE BEING PROVIDED BY GOLD AND GRACE, LLC AT MY REQUEST, AND THAT THIS WAIVER AND CONSENT IS A MATERIAL INDUCEMENT FOR GOLD AND GRACE, LLC TO PROVIDE ITS SERVICES.
